West Virginia man arrested in Logan County fake prescription case

A West Virginia man is facing several charges after authorities say he attempted to use a fake prescription to obtain a controlled substance from a West Liberty pharmacy.

According to the West Liberty Police Department, officers responded to C&R Pharmacy on March 18, 2026, around 5:35 p.m. after pharmacy staff reported a suspicious prescription request.

Pharmacists told officers a man later identified as 40-year-old Douglas Boe of West Virginia was attempting to pick up a prescription for Phenergan with codeine, a Schedule V controlled substance.

Police say Boe told officers he had traveled from West Virginia to pick up the prescription for a friend, but refused to identify the person.

He reportedly claimed he had been dropped off across the street from the pharmacy, but could not describe the vehicle or driver.

During the investigation, officers say Boe voluntarily handed them an Ohio temporary driver’s license bearing the name “Jason Alan Smith.”

Boe allegedly admitted the document was fake and said a friend had provided it so he could retrieve the prescription.

Pharmacy staff told police the prescription had been called in roughly 90 minutes earlier by someone claiming to be “Doctor James Armstrong.”

After contacting the listed medical office in Kentucky, staff learned no such prescription had been authorized, and there were no patient records for Jason Alan Smith.

Investigators also discovered the DEA number used during the call was invalid, though the NPI number provided was legitimate.

Security footage reportedly showed Boe entering the pharmacy, though the vehicle that dropped him off was not captured on camera.

According to Logan County court documents, an indictment against Boe was filed on April 14th, and a warrant was issued later that same day.

Although the incident happened in March, Boe was not arrested until Tuesday afternoon.

He was lodged in the Logan County Jail shortly before 4 p.m. on charges of forgery, attempt, and illegal processing of drug documents.

Boe is scheduled to be arraigned Wednesday afternoon in Logan County Common Pleas Court.
